Registration Packets will be available to pick up in the Sunrise office from March 1st to 24th The packets can be returned during our regular office hours 8 am to 4 pm We will also be open March 15th until 4:30 pm and March 24th from 7:00 am to 4:30 pm Children need to be five years old before September 1, 2011. If you plan to register in the office, please bring the following with you: 1. Birth Certificate 2. Proof of Residency – a phone or utility bill 3. Immunization Dates 4. Home, Work and Cell Phone Numbers 5. Emergency Contact Information 6. Doctor’s and Dentist’s Name and Phone Number 7. Daycare Information March 25th is the last day to turn in completed registration packets to be included in the fullday kindergarten lottery, if more than 24 students register for full day.
